MedSpa Insurance
Medical spa and aesthetics insurance in North Carolina
NC's medspa market is booming, Raleigh, Charlotte, and Wilmington all have dense concentrations. This is a high-risk class: medical procedures in a spa setting create significant liability that standard business coverage does not address.

Professional liability / malpractice
The most critical coverage for any medspa. Covers claims arising from Botox complications, filler injuries, laser burns, chemical peel injuries, and other professional errors. One serious outcome can generate a six-figure claim. GL does not cover this, a separate professional liability policy is required.
General liability
Covers premises liability, a client slipping on a wet floor, a visitor injured in the waiting area. The foundation policy, but not sufficient on its own for a medspa.
Commercial property
Covers the building (if owned), tenant improvements, and equipment. Laser machines cost $80,000 to $200,000 or more. Make sure equipment is covered at replacement cost, not depreciated value.
Medical equipment coverage
Standard commercial property may have sublimits for electronic equipment. Confirm that your high-value laser and aesthetic devices are specifically scheduled and insured at full replacement cost.
Cyber liability
Medspas collect patient data subject to HIPAA. A breach triggers notification requirements, regulatory exposure, and reputational damage. Cyber liability covers breach response costs, notification, regulatory defense, and first-party losses.
Workers' compensation
Required in NC with 3 or more employees. Covers on-the-job injuries to staff, nurses, aestheticians, front desk.
The medical director requirement
NC medspas must operate under a licensed physician medical director. The physician's professional liability (malpractice) coverage and the medspa's professional liability policy are separate. The physician is covered for their individual clinical decisions and supervision responsibilities. The medspa entity is covered for the business operations and the employees acting under supervision. A gap in either exposes the operation to uncovered claims.
Laser and injectable underwriting
Medspa underwriters ask detailed questions: what procedures are performed, by whom, and with what credentials. Botox and fillers are the most common procedures and the most common source of claims. High-powered lasers, fractional CO2, Nd:YAG, IPL, create additional risk when operators lack documented training and certification. Some carriers decline coverage for specific procedures entirely or exclude them. Carriers that specialize in the medspa class understand the nuances; generalist carriers often do not.
Cooper's take
Medspa is one of the hardest classes to place correctly. Rates have increased sharply due to claim frequency, and coverage terms vary significantly between carriers. You need a carrier that understands the class, not a standard business package that excludes professional liability and does not account for the medical nature of what you do. Get this wrong and your policy will not respond when it needs to.

Frequently asked questions
What is the most important insurance coverage for a NC medspa?
Professional liability (malpractice) is the most critical coverage for a NC medspa. A Botox complication, filler migration, laser burn, or chemical peel injury can generate a six-figure claim. General liability alone does not cover professional errors and omissions, a separate professional liability policy is required.
Does the medical director need separate insurance from the medspa?
Yes. The medical director's professional liability is separate from the medspa's general liability. The medspa's policy covers the business entity; the physician's malpractice policy covers their individual professional decisions and supervision. Both are needed. A medspa without a properly insured medical director is exposed on both fronts.
Do NC medspas need cyber insurance?
Yes. NC medspas collect patient data subject to HIPAA, names, dates of birth, treatment records, payment information. A data breach triggers HIPAA notification requirements, potential regulatory fines, and reputational harm. Cyber liability coverage pays for breach response, notification costs, regulatory defense, and first-party losses. General liability does not cover cyber events.
How does laser equipment affect medspa insurance premiums?
Underwriters ask specifically about laser equipment, type, power level, and the credentials of operators. High-powered medical lasers (fractional CO2, Nd:YAG, IPL) used by insufficiently credentialed staff elevate the risk profile and can affect both premium and coverage terms. Some carriers will decline to cover certain laser procedures without documented provider training and certification.
Why is medspa insurance more expensive than a standard spa?
Medspas perform medical procedures in a non-hospital setting. Botox, fillers, laser resurfacing, and chemical peels carry meaningful complication rates and can cause permanent injury. Claim frequency in the medspa class has increased significantly over the past decade, driving premium increases across most carriers. The class is considered high-risk by most standard commercial insurers.
